I acknowledge that participation in Pilates, fitness classes, or private training sessions at Wake Pilates Company involves physical activity that may be strenuous and could cause injury. I voluntarily assume full responsibility for any risks, injuries, or damages, known or unknown, that I may sustain as a result of my participation.
2. RELEASE OF LIABILITY
In consideration of being allowed to participate in activities at Wake Pilates Company, I hereby release, waive, discharge, and covenant not to sue Wake Pilates LLC, its owners, officers, employees, contractors, and agents from any and all claims, demands, damages, or liabilities arising out of or related to my participation in activities, including but not limited to:

I understand that Wake Pilates Company staff are not healthcare professionals and cannot provide medical advice. I certify that:
I am in good physical health and have no medical condition that would prevent my participation in the activities offered.
I will inform Wake Pilates Company of any pre-existing conditions or injuries that may affect my ability to participate.
I will seek professional medical advice before participating if I have any health concerns.
4. CONSENT FOR EMERGENCY TREATMENT
In the event of a medical emergency, I authorize Wake Pilates Company to provide or arrange for first aid and seek medical treatment on my behalf. I agree to bear all costs of such treatment.
5. CANCELLATION AND REFUND POLICY
I understand and agree to Wake Pilates Company's cancellation and refund policy as stated in their official documentation or website.
6. ACKNOWLEDGEMENT OF UNDERSTANDING
I have read this waiver and release of liability, fully understand its terms, and acknowledge that I am signing it voluntarily. By signing, I agree to waive substantial rights, including my right to sue.
